Tennessee Gov. Lee signed HB0754 requiring providers to report transgender patients' data to the state

Governor Bill Lee signed House Bill 0754 into law on May 15, 2026, when it was assigned Public Chapter 932. The Tennessee General Assembly had passed the measure earlier that spring, with the House concurring 73-23 and the Senate approving it 24-7. The bill's lead sponsor was Rep. Jeremy Faison, and its reporting framework closely tracks model legislation promoted by the anti-transgender advocacy group Do No Harm.

The law requires healthcare providers and insurers that receive state funding to submit information to the state about patients who receive gender-affirming care. Reportable data includes the type of transition-related procedure, the patient's sex assigned at birth, age, county of residence, the provider's name and contact information, and documented neurological, behavioral, or mental health diagnoses. Providers who fail to comply face suspension of their license for at least six months, investigation by the state, and fines of up to $150,000. The law separately requires people seeking insurance coverage for gender-affirming care to also purchase coverage for detransition.

Supporters, including Faison, characterized the reporting requirement as a way to study treatment trends and outcomes without identifying individual patients. Transgender residents, medical providers, and civil-rights advocates described it instead as a state surveillance registry that assembles sensitive medical information about a population defined by identity, warning that the reported data could be used to target transgender Tennesseans. A separate 2026 Tennessee measure addressing prison and facility placement for transgender people, HB0571, is distinct from this reporting law.
